Scope:. Horizon shall provide to school on an as needed and as-requested basis, the full range of staffing services including Registered Nurses, Licensed Practical Nurses, Certified Nurse Assistants, Health Aides, and other Clinical professionals hereinafter referred to as ("Clinicians"). Additional practitioners (and or other temporary staff) and associated rates can be added as an addendum to this contract with agreement of other parties.
Additional funding is needed to cover the cost for schools that are lacking nurses.

WHEREAS the Board of Education wishes to obtain nursing services for the District,
WHEREAS the Board has selected Horizon Healthcare to provided these services
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorized the Superintendent of Schools to enter into a contract with Horizon Healthcare Staffing in an amended amount not to exceed $33,440.00.
