Term: 2018-2019

Amount- $16,325.00
Scope-Comprehensive assistive technology evaluations will be done by the Westchester Institute for Human Development
Assistive Technology Team. The comprehensive evaluation may require the services of one or more of the following disciplines: Speech-Language Pathologists (Augmentative/Alternate Communication/Instructional Technology); and/or Occupational Therapists (Access/Instructional Evaluation fee will be determined as follows:
One discipline $1,380.00
Two discipline $1,890.00
For AAC Evaluations-price includes one hour of set up by an ACC speech expert.
Travel Cost per school visit $25.00 under 20 miles/$50. - over twenty miles.
There will also be an additional charge of $200 in the event and evaluation has to be re-written (in medical or educational format)
Westchester Institute for Human Development will also provided the following training:
Assistive Technology training for student and Five (5) hours training conducted by an OT therapist.
$225.00 per hour or $560.00 for 3 consecutive hours.
Travel fee: $25 under 20 miles/$50 over 20 miles.
Services shall be provided on an as needed basis as requested by the District.

Resolution:

WHEREAS the Board of Education of the City of Yonkers is required to provide services to the students of the Yonkers Public School District, and
WHEREAS these services will be provided by the Westchester Institute for Human Development, and
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into an agreement with the Westchester Institute for Human Development in the amount not to exceed $16,325.00
